Need to make a change to an existing appointment? No problem, we have several options available to you.

1. If you wish to reschedule or cancel your appointment with at least seven calendar days between the date you reschedule/cancel and the date of your original appointment, you can use our online Appointment form to request a new appointment date or notify us of the cancellation.
2. If there are fewer than seven calendar days between the date you reschedule/cancel and the date of your original appointment, we ask that you notify us by phone.
3. Please remember, in the event that is necessary for you to change your appointment, we require 72 hours advance notice. This will allow time for another patient in need of care to be seen.
